A: We are located at the corner of Highway 6 and University. Our address is 800 Bonaventure Way, Suite 160, Sugar Land, Texas, 77479. We are located in an office park known as Telfair Office Park. We are located close to several different imaging centers should imaging be a part of your treatment plan. This enables you to quickly have these procedures performed without worrying about running all over Houston. Q: How much does chiropractic care or therapy cost?

A: There are several factors which go into determining what the cost for your treatment will be. First, is if you are using your health insurance. When electing to use your health insurance, our staff at Restoration Health will call your insurance carrier to verify what your out of pocket costs will be based on your plans coverage. Each patient and each insurance have differences. Some plans have you only pay a copay to cover your treatments, while other plans have you pay a percentage. These payments further depend on what your deductible is and whether you have met this deductible at the time of treatment.

Patients electing not to use their health insurance or are not covered by a health insurance plan can take advantage of our cash prices. For the most up to date pricing for treatment, please call Restoration Health and our staff will be happy to provide this information and answer any other questions you may have.

A: Yes! Restoration Health has always and will always, continue to protect your privacy at all times. The HIPAA (Health Information Portability and Accountability Act) guidelines which govern your right to privacy will always be followed to ensure you have peace of mind when it comes to your health information. What this means to you, is that should anyone request your health information a HIPAA release form will be required before Restoration Health releases any records. This form will have been signed and dated by you, so you have final say on where your records are going

Furthermore, you can rest assured that should you desire for your records to be released, Restoration Health will respond quickly to complete this request. We value your health, and we will submit your records to any provider you desire should a HIPAA release be issued.

Q: What is required for my initial visit?

A: The simple answer for this question is you just need to bring yourself.

For a more indepth answer, the requirements will depend on what is bringing you to our Sugar Land Chiropractors office headquarters.

If you are not planning on using your health insurance and you would prefer to pay by check or cash, we do require a state ID or a federal picture ID to treat you as a patient legally.

If you are a patient that plans on using your health insurance, you will also need a federal or state picture ID card and you will also need to bring your health insurance card which will be required by law.

For patients involved in a car accident or personal injury care, a state or federal picture ID card and information you have regarding the accident will be required. This may include, but is not limited to, car insurance information for yourself, car insurance information from any other drivers involved in the accident, attorney information from any representation you may have, and a police report should you have access to one.

For patients injured on the job or worker’s compensation related appointments, you will need all of the following: 

  • A state or federal picture ID card 
  • Any information you have regarding the accident
  • Attorney information from any representation you may have. 
  • Any information your company presents you will be required. This may include, but is not limited to:
    • A Worker’s Compensation or Worker’s Injury Packet
    • The insurance information from the Worker’s Compensation insurance your company has
    • Any forms which were provided to you upon your injury which you have completed and signed
    • The forms mentioned above, are typically numbered and may include but are not limited to:
      • Form 41 – which is an incident report from when you were injured
      • Form 73 – a work status form which will be completed by your treating doctor or any specialists you have seen
      • Form 53 – which is a change of treating doctor if you are attempting to switch doctors.
